What is Heart Attack? Causes and Effects — Heart Care Pack

 Abstract

The heart is one of the most important vital organs of the body. The heart makes sure to ensure the survival of each and every cell of the body, by circulating essential oxygen and nutrients. The heart is composed of muscle cells called cardiomyocytes and consists of four chambers 2 auricles and 2 ventricles. The heart receives special blood vessels called coronary arteries for the blood supply. Impaired blood supply to the heart may cause the death of heart cells. In general, the death of heart cells may lead to heart disease or heart attack. This article contains information about heart attacks and associated information.

Heart Attack

Introduction

A heart attack is also called a myocardial infarction. It happens when the heart muscle is not able to get enough blood. If left untreated, gradually it may lead to heart muscle damage. Excessive intake of trans fats, smoking, and alcohol abuse may lead to atherosclerosis. Atherosclerosis is a degenerative disease in which lipid deposits or plaques appear in the layers of arteries. Atherosclerosis is also one of the biggest reasons which cause of heart attacks. The exact term for this is coronary artery disease. Simply we can say that if there will be an obstruction in the blood flow of the heart, it may lead to heart disease.

Causes

The leading cause of heart attacks is coronary heart disease. This is where plaque builds up in the arteries that supply blood to the heart. The general buildup of plaque in the arteries is also known as atherosclerosis.

  1.  Blockage–Heart attacks are prominently associated with the blockage of arteries. Excessive consumption of junk food and poor lifestyle leads to the formation of plaque which leads to blockage of arteries hence causing heart attack.
  2.  Lack of oxygen–Heart attacks may be caused in case the heart doesn’t receive oxygen-rich blood. This may be associated with coronary artery diseases.
  3.  Blood vessel rupturing
  4.  Spasms in blood vessels
  5.  Drug abuse
  6.  Hypoxia or low level of oxygen in the blood
  7.  Hypertension
  8.  Raised levels of blood cholesterol
  9.  Smoking
  10.  Associated conditions like kidney issues, congenital issues like bicuspid heart valves
  11.  Poor lifestyle
  12.  Age and family history can increase your risk for heart disease and heart attack.
  13.  Diabetes
  14.  Hereditary
  15.  Obesity
  16.  Lack of physical work
  17.  Sedentary lifestyle

Symptoms of heart attack

The major symptoms of a heart attack are

  1.  Chest pain or discomfort– Chest pain is one of the specific clinical features of a heart attack. Chest pain is associated mostly with the left or center of the chest. Pain is usually like pressure, fullness, or squeezing pain.
  2.  Cold sweat, weakness
  3.  Pain or tingling sensation in the jaw, back, or neck
  4.  Pain in arms or shoulder
  5.  Breathlessness or shortness of breath
  6.  Tiredness or giddiness
  7.  Weight loss
  8.  Nausea
  9.  Vomiting
  10.  Change in complexion
  11.  Pallor

With Ayurvedic correlation of symptoms

  1. Cyanosis (vai varna)— due to poor blood circulation, bluish discoloration of the skin occurs.
  2.  Syncope (murchha)— loss of consciousness due to the impaired blood supply to the blood.
  3.  Fever (jwara)— fever and feeling low
  4.  Cough (kaas)— due to some underlying condition or build \ of fluid in lungs.
  5.  Hiccup (tikka)–hiccups sometimes are associated with heart failure.
  6.  Dyspnoea (swash)— lack of breath in enough air
  7.  Tastelessness or bad taste (asya-vairagya)— bad taste can be associated as a symptom.
  8.  Excessive thirst (trisha)— due to aggravated pitta there may be an increase in Pitta dosha and hence an increase in thirst can be there.
  9.  Drowsiness (pramoha)— drowsiness may be due to aggravated vata
  10.  Vomiting (chhardi)— the aggravated pitta or kapha may cause vomiting.
  11.  Excessive salivation (kaphotklesha)-– as the name indicates this symptom is associated with aggravated kapha dosha.
  1.  Chest pain (Ruja)— aggravated vata may cause throbbing pain in the chest region.
  2.  Loss of appetite (aruchi)— Loss of appetite may be associated with an imbalance in pitta dosha.

Effects and consequences

There has been a rise in the incidents of heart attacks. Changes in lifestyle in urban areas, and sedentary work culture lead to increased incidents. The ultimate consequence of a heart attack for an individual is dead. But persons associated with that individual suffer other consequences also. Although any individual may suffer from a deadly heart attack, we have seen that it is prevalent much in high-class people, and business persons. The reason behind it is a sedentary lifestyle that is devoid of exercise and walking. Such individuals pay heavily for their medications and treatment. Their families suffer from the loss of a loved one, financial losses, and emotional breakdowns. A person with a family history of hypertension always feels tense. So it is the demand of time to get rid of a sedentary lifestyle, maintain a healthy weight, and eat moderately.

Management

The management of Heart attack consists of the administration of medications. These medications include antiplatelet agents, statins, and beta blockers.

Antiplatelet agents

Aspirin– Aspirin interferes with homeostasis by inhibiting platelet aggregation which is due to TXA2 production. Before it is deacetylated in the liver, aspirin acetylates COX in platelets while they are in portal circulation. Further, platelets are unable to regenerate COX (lack nucleus: do not synthesize fresh protein), while the vessel wall is able to do so (fresh enzyme is synthesized within hours).thus in normal dosage aspirin inhibits TXA2 production and promotes antithrombotic effect. Hence lower the chances of conditions like a heart attack. Aspirin lowers blood clotting hence it promotes blood flow via narrow arteries. Dipyridamole – It is a vasodilator that inhibits phosphodiesterase and blocks the uptake of adenosine to increase platelet cANP which potentiates PGI2 and interferes with aggregation. Dipyridamole has also been used to enhance the antiplatelet action of aspirin. Statins – Drugs like atorvastatin control the cholesterol levels in the body. Statins also help to lower the levels of bad cholesterol. Beta-blockers – Beta blockers are useful to relax the muscles of the heart. These drugs help to lower the heartbeat and control blood pressure. Also, beta blockers are helpful to control the damage to heart muscles and help to prevent further heart attack incidents. Atenolol is a potent beta-blocker.

Ayurvedic treatment is based on the management of tridoshas. Vataja heart disease can be managed with herbs like Devadaru (Cedrus deodara), Punarnava (BoerhaviaDiffusa), Kulattha (Dolichos biflorus), Bilva (Aegle marmelos), Indian gooseberry and jaggery. In pittaj heart diseases herbs like Katphala (Myrica nagi), Yashtimadhu (Glycyrrhiza glabra), honey, and Arjuna (T. arjuna) are very useful. In Kapha-associated heart disease Haritaki (Terminalia chebula), Ginger (Zingiber officinale), Ativisha (Aconitum Heterophyllum), Devadaru (Cedrus deodara). These herbs as mentioned in the texts are beneficial to maintain the heart rate, strengthening the muscle tissue, and helping to treat the affected doshas.

It is important to lead an active life with a good workout according to the body’s BMI. Avoid virudhaharam (incompatible diet) and take the diet according to the season.  Lead an active life with workouts and exercise. Avoid negative thoughts and stress. Yoga, dhyan, and pranayam should be part of daily life. Kaphaj Sahara vihar should be avoided in case of high triglycerides.

Ayurvedic aspect

From the ayurvedic perspective, we can correlate heart issues with Haridyarog. Vyana Vata (a type of vata), sadhana pitta (a type of pitta that resides in the heart), and avalambaka kapha (a type of kapha) are associated with the heart. These are the types of vata, pitta, and kapha residing in the heart. Rasa, rakta, muscular tissue, oja, Pran Vayu (oxygen), vyana vayu (autonomic nervous system & central nervous system), sadhana pitta and avalambaka kapha are the basic elements, which maintain the integrity of the cellular structure and various functions of the heart. Factors that affect these basic elements can lead to heart disease. Heart disease can be associated with a complication of other diseases.

The classification of Hriday rog is as below

  1. Adibalapravrutta (Hereditary heart disease)
  2. Janambalapravritta (congenital heart disease)
  3. Doshabalapravrutta (Vataja, pittaja, Kaphaja, sannipatika)
  4. Sanghatabalapravrutta (Traumatic, poisoning)
  5. Kalalbalapravrutta (Environmental seasonal)
  6. Swabhava Balakruta (Ageing, Death)
  7. Adhyatmika (Psychological factors leading to heart disease)
  8. Daivabalakruta (idiopathic, bad luck due to bad deeds of previous lives)

Another concept associated with the heart chakra is associated with energy. The heart chakra can be considered a junction between the inner and outer world. Heart energy is associated with love feelings, loneliness, sadness, and grief. Hence the heart chakra is a nerve plexus, and the number of energetic pathways meets in this chakra. These pathways enhance energy and hence help to promote heart health.

Management of Mucormycosis in Dogs Through Ayurveda

ABSTRACT

There are various types of fungi present in the soil. The dogs usually spend a lot of time in mud and soil. This makes them more prone to catching infections and various pathogens. The mucormycosis is one such type of fungal infection which will be discussed in detail in the following article.

INTRODUCTION

Mucormycosis is a combination of mold and fungal infections and diseases. The mucormycosis that infects the dogs will be explained in detail in the following article. The mucormycosis usually targets the skin along with the gastrointestinal tract. The dogs usually get contaminated from this infection through the medium of soil. When the fungus comes in contact with an injured dog, it enters the body and spreads rapidly. If an infected dog comes in contact with a human, the mucormycosis infection can spread to humans also. This infectious pathogen spreads rapidly in the body. It starts invading the walls of the blood vessels. The damage to blood vessels lead to reduced blood flow. This condition needs immediate diagnosis and proper management. A delay in medical intervention may result in extensive damage to the body and eventually lead to the death of the dog.

CAUSES

There are various classes of fungi present in the surrounding environment. One such class of fungi is mucormycosis. It causes zygomycetes, etc. It is found all over the world. It usually feeds on dead and decaying matter. It dwells within soil. It can transfer from the soil to the animal dander (hair). Then this fungus infects the dog and spreads rapidly in its body. Eventually damaging the blood vessels and blocking the blood flow. If left unattended, this condition proves to be lethal.

SYMPTOMS

The symptom of every disease differs from patient to patient in humans. Similarly, in dogs also, the symptoms will differ from individual to individual. The most common symptoms are as follows – vomiting, jaundice, pain in affected areas, etc. Various respiratory problems can also be seen such as pneumonia, etc. There may be development of various nodules in the body. The symptoms will depend on the severity of the condition. The strength of the immune system of the dog will also determine the presentation of the features. This condition should be diagnosed and managed in a timely manner. Otherwise, the consequences may be lethal.

DIAGNOSIS

The diagnosis of the condition of mucormycosis in dogs will include both the clinical examination and the laboratory diagnosis. The clinical examination will include history taking, and observation of symptoms, etc. The history taking will include information regarding the recent diet routine of the dog and possible sources of exposure in recent times. The lab diagnosis will include various lab tests such as urine analysis, feces examination, biochemistry reports, etc. A biopsy may also be required to support and confirm the diagnosis.

TREATMENT

The management of the mucormycosis will depend on the condition of the dog. The usual treatment of mucormycosis will include administration of antifungal drugs and anti – biotics. Severe cases may need hospitalization also. This condition should not be ignored and needs timely management.

AYURVEDIC ASPECT

The mucormycosis in dogs occurs due to infiltration of the causative factors and resulting in an imbalance of the internal environment. The nidan (causes) lead to disturbance in the tridosha (vata, pitta, kapha) levels of the body. So, the aim of management will include removing the nidan (causative factors) and balancing the levels of dosha through administration of ayurvedic formulations.
