Abstract

CHYLOUS ASCITES occur when chyle collects in the belly. Chyle is a milky fluid which is made in the intestines. It is an uncommon and a rare type of condition arising from obstruction of lymphatic drainage or trauma. It was first reported in 1912. It has an incidence of approximately 1 per 20,000 people. The present ascitic fluid  can turn into chylous fluid as a secondary event. It can occur in children, although most are infants and toddlers and adults.

INTRODUCTION

Chylous ascites is defined as the milky appearance that results from leakage of lipid rich lymph into the peritoneal cavity. This usually occurs due to trauma and increased peritoneal lymphatic pressure or rupture of the lymphatics. Presence of the ascitic fluid with high fat (triglyceride), usually more than 200 mg/dL is defined as true chylous ascites.

SYMPTOMS

Chylous ascites are usually asymptomatic, but the following symptoms can be seen.

  1. Weight gain
  2. Shortness of breath
  3. Increased abdominal pressure
  4. Enlarged lymph nodes
  5. Malaise
  6. Anorexia
  7. Vomiting or Diarrhea
  8. Abdominal pain due to sudden leakage of chyle into the peritoneum.
  9. Bulge or swelling

CAUSES

  1. Injuries due to Trauma or surgery
  2. Rupture of lymphatics
  3. Increased peritoneal pressure
  4. Obstruction
  5. Malignancy
  6. Congenital lymphatic malformations
  7. Systemic infections such as tuberculosis, It can also occur from Cirrhosis,  pericarditis etc
  8. Inflammatory causes such as peritonitis .
  9. Mediastinal tumors

Diagnosis of chylous ascites

  1. Paracentesis – sample of the fluid is taken from the belly with the help of the needle and examined under the microscope.
  2. Ultrasound
  3. Lymphoscintigraphy
  4. Magnetic resonance (MR) lymphography
  5. Blood culture – triglyceride test
  6. CT ( computed tomography )

MANAGEMENT

As per modern : The management will be in two types : Conservative and Surgical.

CONSERVATIVE -Low fat diet

  • IV nutrition
  • Removing fluid from the belly ( Paracentesis)
  • Placing a drain in the belly to let fluid flow out.
  • Transjugular intra hepatic porto systemic shunt (TIPS)

Introduction

Ascites is a condition in which there is an accumulation of free fluid in the peritoneal or abdominal cavity, which gives a feeling of fullness and rapid weight gain. Ascites is categorized mainly into two types depending on the protein content in the fluid:

  • Exudate: Exudate has a protein content of more than or equal to 2.5g/dl.
  • Transudate: Transudate has a protein content of less than 2.5g/dl.

CAUSES OF ASCITES

Cirrhosis of the liver is the most common condition that can cause ascites. Other conditions that cause ascites can be heart failure, kidney failure, infections (such as tuberculosis ),or
cancer.

How does cirrhosis cause ascites?

When a patient has cirrhosis, his liver doesn’t function normally as it should. The abnormal liver function combines with portal hypertension to cause ascites. This portal hypertension is the high pressure in portal veins (the veins that supply blood to the liver). This abnormally high pressure in portal veins causes fluid to leak out of the veins into the belly and collect there as a fluid.

RISK FACTORS FOR ASCITES

Conditions causing cirrhosis of the liver puts a patient at a greater risk of developing ascites. These conditions are Non-alcoholic fatty liver disease, Hepatitis-B, Hepatitis-C, Autoimmune hepatitis, Alcohol use disorder, Genetic liver diseases that may include Wilson disease and alpha
