Abstract

Southern tick associated rash illness is an emerging infection which is related to lyme disease. It occurs in the southeastern and central parts of the United states. This condition is also spread by ticks bite and according to a hypothesis the tick bite transfers bacteria Borrelia lonestari through the bite. But there is no evidence to declare that this condition is caused by the borrelia or there is no evidence that states that borrelia strain as a causative agent. Southern Tick Associated Rash Illness

Introduction

During 1980’s, the physicians practicing in the southeastern United states described a new illness which is characterized by the presence of a rash which is typical of erythema migrans and a mild flu like symptoms that was temporally associated with the bite of a lone star tick.over the next 20 years, the illness was given variety of names which includes southern lyme disease and masters disease. But the most widely used name is, southern tick associated with rash illness. At present the disease is considered as the syndrome of unproven cause that is clinically diagnosed on the basis of its characteristic skin rash, mild clinical course and occurrence in the areas where lyme disease is uncommon or not present.

Causative Organisms

Southern tick associated rash illness was presumed to be transferred to humans by lone star tick. But available data do not support this but mentions borrelia lonestari as the causative agent.

Signs and Symptoms

The rash that is associated with Southern tick associated rash illness is circular or elliptical in nature. It is a red coloured rash that typically appears within 7 days of tick bite. In addition symptoms include the following:

  • Fever of high grade
  • Headache, which sometimes is present and sometime its absent
  • Stiffness in neck
  • Joint pain of mild variety
  • Muscle pain

The rash is generally like a bulls eye rash at the site of infection and is called erythema chronicum migrans. Thi si similar to the rash which is found in Lyme disease. Though its symptoms are mild and resemble influenza.

Diagnosis of Southern Tick Associated Rash Illness

For diagnosing this condition there is no serological test that exists. The reason is the unknown etiological agent. Therefore, the diagnosis is generally done on typical rash appearance. The southern tick associated with rash illness is smaller, less variable in shape and has more central clearing.

Treatment of Southern Tick Associated Rash Illness

Generally after the bite from a tick, an individual is kept under observation or is watched closely for at least 30 days. Rest is as following:

A single dose of antibiotic doxycycline is given to the person whose being bit. But the antibiotic dose depends on certain factors like:

  • Person should have the tick attached to his or her body who can spread lyme disease.
  • The tick should be attached to the individual for at least 36 hours.
  • The person should take antibiotics within 72 hours of tick removal.
  • The individual should be 8 years or more in age and should not be pregnant or breastfeeding.
  •  In such cases a days to 4 week course of antibiotics is used to treat people who are being diagnosed with Lyme disease.

Common Choice of Antibiotics

Doxycycline, Amoxicillin, Azithromycin, Cefuroxime and Ceftriaxone. Some painkillers along with muscle relaxants for joint pain and stiffness are also being used.

Complications

  • Chronic fatigue: the person may be left with lifelong susceptibility to get fatigued sooner.
  • Arrhythmia: irregular heart beat may be observed (or not observed) even after relief from the actual ongoing infection.
