ABSTRACT

Astheno teratozoospermia is a disease which comprises two different conditions i.e teratozoospermia & asthenozoospermia. Teratozoospermia is defined as a condition in which there is a high amount of abnormal shaped sperm whereas conditions with low sperm motility and increased viscosity are kept under asthenozoospermia. This is a commonest cause of male infertility. Along with this patient might have oligospermia i.e low sperm count. Aesthenozoospermia can be complete or partial due to which there is a decrease in the quality of sperm. Whereas teratozoospermia is abnormal morphology or shape of the sperms due to which sperm loses the ability to move properly resulting in decreasing the chances of  ovum fertilization.

INTRODUCTION

As already mentioned in the abstract this condition “Astheno teratozoospermia” is a sexual disorder in males affecting the sperm quality which leads to the condition of  infertility. But the quantity of sperm is not hampered in this condition. Astheno teratozoospermia is a condition with reduced motility and abnormal shapes of sperms. Mens with PCD(primary ciliary dyskinesia) have more chances of suffering from teratozoospermia. Teratozoospermia also has 3 types- mild, moderate, severe. This classification is on the basis of percentage of abnormality in sperms. Asthenozoospermia is a condition in which there is decreased motility of sperms and this condition is often accompanied by oligospermia (low sperm count) which is termed as oligo asthenozoospermia. If in a sperm sample 40 percent of sperms are immotile then the diagnosis is asthenozoospermia.

These two conditions mainly have same causes

  9. Bacterial infection

  12. Exposure to toxic substances
  13. Wearing too much tight clothes

2. ASTHENOZOOSPERMIA

  1. Varicocele
  2. Inflammation
  3. Excess stress
  4. Vitamin deficiency
  5. Excess smoking and drinking
  6. Poor diet

SYMPTOMS

Both the conditions have no major life threatening symptoms. Inability to fertilize an egg is the only symptom in both the conditions.

GRADING OF ASTHENO TERATOZOOSPERMIA

1. ASTHENOZOOSPERMIA

  1. A- Rapid progressive motility
  2. B- Sluggish and slow forward progression
  3. C- Non progressive motility
  4. D- No motility

2. TERATOZOOSPERMIA

  1. Mild – 10-14 % sperms have normal shapes
  2. Moderate – only 5-9% of sperms have normal shapes
  3. Severe –  only 5% of sperms have normal shapes

DIAGNOSIS

For diagnosis of both the conditions normally all the tests are same

  1. Semen analysis
  2. Blood tests
  3. Scrotal Usg
  4. Hormonal tests

Asthenoteratozoospermia normally can be diagnosed with semen analysis to check motility and shape of the sperms. If 40% of the sperms are immotile and 96% of the sperms have abnormal shape then the sample of  sperm is diagnosed to be an asthenoteratozoospermic sample.

AYURVEDIC OVERVIEW OF ASTHENO TERATOZOOSPERMIA

This condition in ayurveda can be correlated with the shukra dhatu dusti(semen vitiation). Main function of shukra dhatu (semen) is in reproduction. Shukra dhatu is in the 10 seats of vitality (dashapranayatanana). Hence shukra plays a vital role in a person’s life. Thus depletion or vitiation of shukra dhatu may result in various problems or even death. The vitiation of channels that carry the shukra dhatu is a reason behind the vitiation of shukra. Also incompatible food, use of excess salt, sour products, dry food consumption, bitter or astringent food intake etc can result in vitiation of doshas which affect the shukra dhatu. Not only eatables but there are psychological causes to this condition i.e anxiety, stress, overthinking, distrust etc. may lead to this condition. Due to above mentioned causes there is vitiation of tri doshas (vatta, pitta, kapha – kinetic, thermal, potential energy) which affects the shukra dhatu resulting in vitiation. Vitiated shukra (semen) may be frothy, a bit dry(less fluidity), slimy, bad smell, discolored etc. Astheno teratozoospermia can be further correlated with “granthi shukra” along with “shukra kshaya’’. All the 3 doshas vitiaion vatta, pitta, kapha are there in this condition hence has to be managed accordingly.
