Personality disorders usually begin in the teenage years or early adulthood and are less obvious in middle age. Personality disorders are mainly of three categories: cluster A, cluster B, cluster C. Paranoid personality disorder comes under cluster A personality disorder. A person’s behaviour is odd or unusual to others. In this article we shall be discussing causes, risk factors, management protocol  in modern as well as ancient systems of medicine.

INTRODUCTION

Paranoid Personality disorder or cluster A disorder is a behavioural disorder in which a person distrusts others and assumes that others may harm or deceive them even when they have no sufficient  justification for their feelings. Person with paranoid personality disorder believes that others are constantly trying to demean, harm or threaten them. Their habits of blame or distrust  may interfere with their ability to form close relationships. They are always in constant doubt. They doubt the faithfulness of their spouse or partner or loyalty of friends. As per ancient Ayurvedic context, paranoid personality disorder comes under Manas Roga. The mansic doshas are vitiated in this disorder that further imbalances the body humours (vata, pitta and kapha) that leads to agnimandya (decrease in digestive power).

AYURVEDIC ASPECT AND MANAGEMENT OF PARANOID PERSONALITY DISORDER

Ayurveda has considered the mental and physical diseases as two separate specialised subjects only in certain contexts. However, no line of demarcation has been drawn between the physical and mental diseases and a flexible psychosomatic approach has been put forward. The Treatment of different psychological illnesses, as described in Ayurveda, reveals that the psychological, spiritual and pharmacological methods have been used side by side to root out the disease.

1. Sattvavajaya Chikitsa

Sattvajaya word means boosting up the satva that is mana (mind) that is the root cause of the disorder. The psychotherapeutic approach to the management has been described under Sattva Vijaya Chikitsa. Aachaya Charaka defines it as a method of restraining or withdrawal of the mind from unwholesome objects (Arthas). It aimed at the control of mind that one should keep himself established in his/her own oneself after knowing the true nature of his/ own soul and attaining the height of spiritualism.

Manaso Jnana, Vijnana, Dhairya, Smrti, Samadhibhih

(Ch. Su. 1/58)

Dhairyatmadi Vijnanam ManodosausadhamParam

According to Aachaya Charak it implies preventing the mind from the desires of unwholesome objects the cultivation of Janan (cognition), Vianan (worldly knowledge), Dhairya (patience), Smrti(recall) and Samadhi (yogic psychotherapy). Samadhi refers to practising yoga and meditation to reduce the waverings of mind ultimately leading to focused and tranquilised mind. All these measures help in developing control over the Manas or mind, which is always unstable.

2. ASHWASAN CHIKITSA

Ashwasan Chikista refers to the word reassurance. It means listening to the patient and counselling him/her accordingly. It relieves fears, boosts self confidence and promotes hope in a patient. The nature of the disease should be explained to the patients freely and frankly, so that the patient develops insight into the disease . Further he should be educated about the necessity of medication and taking treatment. Education about the disease should also include the reasons for relapse of the disease, so that they could be prevented by adopting appropriate measures.
