Natural Treatment of Primary Biliary Cholangitis
Abstract
Despite sky touching technologies for the treatment of disease in modern science, there are many diseases that have no medical cure. Such diseases are managed by prophylactic treatment. But even after prophylactic treatment the disease never stops growing. It grows till the stage of organ failure and a transplant is required. Sometimes conditions get so worse that even transplanted organs fail and ultimately death. One such disease is primary biliary cholangitis. This disease destroys the bile channel inside the liver. Destruction of bile channels inside of the liver results in accumulation of bile in it and thus results in cholangitis.
Introduction
In primary biliary cholangitis, biliary means related to bile duct and cholangitis means inflammation of the bile duct system. It is also termed as primary biliary cirrhosis. The main cause for this condition is damage to bile duct or bile channel due to bacterial infection or by autoimmune process. The biliary system is responsible for carrying bile to the small intestine in order to complete the digestion process. Bile also helps our body in getting rid of toxins, cholesterol and residues of red blood cells. In ayurveda this condition can be best correlated as yakritpleehodra, a subtype of udra roga in Charaka chikitsa.
Stages of primary biliary cholangitis
The progression of this disease can be further categorized into four stages, these are:
Stage 1: (Florid)
Bile duct damaged due to inflammatory actions.
Stage 2: (Periportal)
Bile duct vanishes, portal or periportal inflammation with ductular proliferation.
Stage 3: (Fibrosis)
Portal fibrosis takes place.
Stage 4: (Cirrhosis)
Irreversible changes occur on and inside the liver surface.
Causes
In modern science only two causes are explained either by bacterial invasion or by the autoimmune process (tendency of body’s immune system to kill its own healthy cells).
In Ayurveda if doshas are too much accumulated inside of body and in addition a person is having poor state of digestion and use hot, saline, alkaline, burning, sour and poisoned food or intake of improper food after evacuative therapy or improper evacuative therapy, suppression of natural urges, morbidity in channels, ama dosha, obstruction due to piles etc. result in udara roga of all types.
Acharayas believed that the accumulated doshas by blocking the channels carrying sweat and fluid and affecting prana, agni and apana produce abdominal diseases (udara roga).
Symptoms of Primary biliary cholangitis
The symptoms initially include fatigue, itching over skin and dry eyes. But with passage of time and ignorance patient many develop severity of symptoms like pain in right upper abdomen, swelling in spleen, fat deposits around eyelids and over skin, yellowish discoloration of skin and eyes, skin darkening due to hyperpigmentation, weak brittle bones, slow buildup of fluid inside body due to liver dysfunction, ascites in case of liver failure and swollen lower limbs or ankles.
The general symptoms according to Ayurveda acharya in all udara roga includes gurgling sound in abdomen, swelling of hands and feet, poor digestion and emaciation (to grow thin), yakritpleehodar (hepatomegaly, splenomegaly), and accumulation of free fluid in abdomen. (due to bile accumulation).
Risk factors
- Smoking
- Drinking alcohol in excess
- Intake of heavy oily meals on regular basis
- If runs in a family (genetic)
- Obesity
- History of any other liver condition
Diagnosis
There are three key points out of which at least two should be fulfilling:
- Persistent raised levels of serum alkaline phosphatase.
- Presence of anti mitochondrial antibodies.
- Liver biopsy histology with persistent primary biliary cholangitis.
Herbal Remedies for Primary Biliary Cholangitis
In Ayurvedic aspect udara roga is the result of excessive accumulation of doshas resulting in obstruction of channels. In this case one should purge the patient daily. 1. Yakritplihantak churna
This blend powder is known for its effect on liver disorders. The ingredients are:
Bhumiamla (Phyllanthus niruri)
Bhumi amla is known by another name as Dukong anak. This herb has properties which can act on oedema in the liver, hyperbilirubinemia like in primary biliary cholangitis. It also detoxifies the liver. Due to its rejuvenating property nourishes the body.
Kalmegh (Andrographis paniculata)
Kalmegh which is also known as kalmegh is known for its hepatoprotective property. It balances pitta and kapha.
Katuki (Picrorhiza kurroa)
Kutaki helps in maintaining proper metabolism of liver and also in eliminating accumulated fat on liver. Regular use of kutki in particular doses helps in eliminating, cleansing and preventing gallbladder stones.
Punarnava (Boerhavia diffusa)

The bhringraj is neetrahitkara (beneficial for eyes), jatharagni deepaka (revive digestion fire), , keshranjaka (hair vitaliser) , pachana (digestion stimulant), kapha visha nasha (eliminate kapha dosha and its toxins from body)shothahara (reduces oedema), rasayana (have rejuvenating properties), pandunut (helps in managing anaemia).
Dosage of Yakritplihantak churna: ½ teaspoon twice daily wit plain water, after meals.
2. Echinacea capsules
These capsules are made up of standardized extract of kalmegh
(Andrographis paniculata). Kalmegh is especially used in managing liver related diseases. Kalmega derived its name from its appearance. Kalmega means “dark cloud”. It has properties to protect the liver from damage caused by free radicals. It is known to be quite effective against hepatitis B viral infection. It is antioxidant, anti-inflammatory, antipyretic, kriminashak, in various skin diseases and has hepatoprotective action.

Bhuminimb is said to be Kalmega’s synonym. The fruit of having a shape like yava. Its taste is tikta (bitter), guna are laghu and ruksha, virya is ushana and action is kapha pitta nasha. It is also believed to be agni deepak (stimulates digestion), veedana nasak (analgesic), kriminashak (kills worms), used in yakrit vikara (liver disorders) and kushta roga.
Dosage: Two capsules twice daily, after meals.
3. Phyllanthus niruri capsules (Bhumiamla)
Phyllanthus niruri capsules consist of bhumiamla alone. It is having therapeutic action on the liver and kidney. It helps in eliminating accumulated toxins from both the organs. Regular use of bhumi amla in jaundice helps in reducing serum bilirubin levels naturally.
